This policy describes how the information you disclose to EZ Vet , LLC may be used and disclosed and how you may access the information. 
EZ Vet Station, LLC collects health information about your pet to allow for consultation and/or treatment through the EZ Vet, LLC and maintains that health information as a personal health record. This Privacy Policy sets forth the practices we follow to protect the privacy and security of your pets health information. We reserve the right to change the terms of this Privacy Policy as necessary and to make the new policies effective for all health information maintained by us. A copy of this privacy policy is always available at each EZ Vet Station.
SECURITY OF INFORMATION
EZ Vet,, LLC is committed to protecting the privacy and security of your information. We backup your pet’s medical summary reports on our private google drive account and store a hardcopy at our corporate office.
USES AND DISCLOSURES OF YOUR INFORMATION
Your authorization. Except as outlined below, we will not use or disclose your information for any purpose unless you have authorized the use of disclosure.  You have the right to revoke that authorization in writing unless we have taken any action in reliance on that authorization.
Uses and Disclosure. We will disclose your pet’s health information to pet care providers furnishing treatment services through EZ Vet, LLC.
We will use your pet’s health information internally for EZ Vet veterinary clinic health care operations and management.
In addition, EZ Vet, LLC may disclose your pet’s health information in the following circumstances without your authorization:

• We may release your pet’s health information when required by law;
• We may release your pet’s health information if required by law to a government oversight agency conducting audits, investigations, or civil or criminal proceedings;
• We may release your pet’s health information if required to do so by subpoena or discovery request.
• We may release your pet’s health information for certain educational/research or marketing purposes as permitted by law.

PROVIDERS
EZ Vet, LLC connects clients to pet care providers who may have their own privacy policies, contained in their Notice of Privacy Practices. As a client you may have certain individual rights regarding your pet’s health information maintained by the providers which is separate from the health information maintained by EZ Vet, LLC, as described in the provider's Notices of Privacy Practices.

RIGHTS THAT YOU HAVE
Access. As an EZ Vet veterinary clinic client, you have the right to a copy of your pet’s health record. If you need a duplicate of your pets medical records you may request it by contacting the EZ Vet corporate office- EZ Vet ,LLC 4866 SW 72 Ave., Miami, FL 33155.
Notice of Breach. We are required by law to notify you in the event there is a breach of security of your information maintained by EZ Vet, LLC resulting in the unauthorized acquisition of your personal information.

Complaints. If you believe the privacy or security of your information has been violated, you can file a written complaint with EZ Vet, LLC, Attn: Compliance Officer 4866 SW 72 Ave.,Miami, FL 33155
